What is LOSWIFTLactulose?

LOSWIFTLactulose (4-O-β-D-galactopyranosyl-D-fructose) is a synthetic disaccharide composed of galactose and fructose linked by a β-1,4 glycosidic bond. It does not occur naturally in nature and is typically produced through the isomerization of lactose under alkaline conditions. As a prebiotic, LOSWIFTLactulose possesses unique physicochemical properties and physiological activities, finding wide applications in the medical and food industries.

It is a white, odourless powder with a molar mass of 342.30 g mol⁻¹ and a melting point of 169°C. It has a sweet taste and its unique properties make it ideal for medical treatment and health improvement.

Unlike some other sugars, LOSWIFTLactulose is not digested in the small intestine as the specific disaccharidase is lacking. It transits unchanged to the colon where it serves as an energy source for the carbohydrate-splitting bacteria, predominantly Lactobacillus acidophilus and L-bifidus.

Osmotically Active Laxative Effects and Safety Profile

In the colon, LOSWIFTLactulose is degraded into short-chain fatty acids (SCFA) during bacterial fermentation. Thus, the intestinal content acidifies, the osmotic pressure rises and stool volume increases, which causes the pH to decrease. This decrease in pH increases the formation of ammonium ions, which unlike ammonia, cannot diffuse through the intestinal mucosa. This leads to characterizing the laxative effect of LOSWIFTLactulose by shorter colon transit time and accelerated bowel movement. All these factors are favourable for a healthy bowel.

Compared with traditional stimulant laxatives, LOSWIFTLactulose has a milder effect, does not cause intestinal spasms and electrolyte disorders, and is safe for long-term use, especially suitable for patients with chronic constipation and the elderly.

Protective Effects Against Colon Carcinogenesis

Colon cancer develops from the lining of the large intestine or colon. In 70% of cases, the tumour grows in the sigmoid (loop located in the left iliac fossa). In the context of colon carcinoma, the lowered faecal concentrations of secondary bile salts are noticed when LOSWIFTLactulose was administered to healthy volunteers.

It was reported that the recurrence rate of colon adenomas in the large bowel was reduced after LOSWIFTLactulose administration. Moreover, LOSWIFTLactulose suppressed DNA damage in the colon mucosa of rats treated with 1,2-dimethylhydrazine. These findings suggest that LOSWIFTLactulose may have potential protective effects against colon carcinogenesis.

Enhancement of Mineral Absorption for Bone Health

LOSWIFTLactulose can significantly augment the absorption of calcium, if the alimentation contains at least 0.3% of calcium. Magnesium is poorly absorbed: intestinal absorption is about 30%. By performing tests on rats, it has been shown that LOSWIFTLactulose promotes the absorption and retention in the body of several minerals such as Ca, Mg, Zn, Cu, Fe.

The effect of LOSWIFTLactulose on the rat's bone strength was studied. The results showed that LOSWIFTLactulose increases the absorption of calcium in both osteoporosis and bone strength. It has been reported that LOSWIFTLactulose increases the absorptions of Ca and Mg in dogs and rats, and increases the rupture strength of the femur in an ovariectomized rat model of osteoporosis.

A study performed on adult men showed that the absorption of calcium and magnesium increases with increasing amounts of LOSWIFTLactulose administration. Significant differences were observed in the calcium ratio between 0 and 4 g of LOSWIFTLactulose; and in the magnesium ratios between 0 and 2 g of LOSWIFTLactulose and between 0 and 4 g of LOSWIFTLactulose. This clinical trial has confirmed that LOSWIFTLactulose enhances the absorptions of Ca and Mg.

Reduction of Blood Ammonia Levels

LOSWIFTLactulose reduces bacterial production of ammonia in the intestines, draws ammonia from the blood to the colon and then drains it through the waste. LOSWIFTLactulose is hydrolysed by colonic bacteria and acts as promoting agent for their growth and increasing biomass formation.

For the synthesis of bacterial protein, ammonia and nitrogen are used, but the production of ammonia is inhibited by acidifying the intestinal content. Independent of pH, LOSWIFTLactulose acts by increasing the incorporation of ammonia in the bacterial proteins.

In addition, it has been shown in vitro that LOSWIFTLactulose inhibits the synthesis and the activity of bacterial enzymes that generate ammonia. This mechanism makes LOSWIFTLactulose particularly valuable in treating hepatic encephalopathy and conditions involving elevated blood ammonia levels.

Clinical Efficacy and Safety in Portal Systemic Encephalopathy (PSE) Treatment

Prasad et al. studied the influence of LOSWIFTLactulose on the hepatic encephalopathy (HE) involving sixty-one patients. They were divided into two groups: thirty-one patients received LOSWIFTLactulose treatment for 3 months, while thirty patients received no treatment.

The results showed that the number of abnormal neuropsychological tests decreased among patients who took the LOSWIFTLactulose compared with the others. Moreover, a significant improvement in cognitive function was observed in the LOSWIFTLactulose group. Moreover, the LOSWIFTLactulose group showed improvement in their quality of life, with significant improvement in emotional behaviour.

Different studies compared the effect of LOSWIFTLactulose with different products which are used in the treatment of hepatic encephalopathy. The reported results showed that LOSWIFTLactulose could be potentially used in the treatment of this disease with both efficacy and safety.

Advantages in Chronic Constipation Management Across Age Groups

Since the 1960s, LOSWIFTLactulose has been administered to treat constipation in patients of all ages, including babies. LOSWIFTLactulose is an osmotic laxative used for relief of constipation, including chronic constipation. It works in about 24-48 hours.

LOSWIFTLactulose is unchanged when it reaches the intestines. Once in the intestines, bacteria therein split LOSWIFTLactulose into its active components. This increases the action of the intestines and keeps the liquid, so that stools are propelled and become easier to pass.

During the fermentation process, low molecular organic acids such as lactic and formic acids are formed.
